Issuing a life certificate

 

The Consulate General of the Republic of Serbia in Sydney may, upon personal request, issue a Certificate of Life as proof of identity and confirmation of life status for the purpose of receiving pension benefits—based on a valid identification document.

Ways to submit the request

1. By post

Send the following:

  1. A completed and hand-signedЗ А Х Т Е В Уверење о животу_3.doc — certified by a Justice of the Peace (JP)

  2. A copy of a Serbian ID card or passport (if you have one) — certified by a JP

  3. A copy of an identification document (passport or photo ID card from Australia or New Zealand) — certified by a JP

  4. Pension Fund (PIO) form (if you have one) — certified by a JP

  5. A self-addressed return envelope with paid postage

? All documents must be certified by a Justice of the Peace.

2. In person at the Consulate

Bring:

  1. A completed and hand-signed З А Х Т Е В Уверење о животу_4.doc (you may fill it in at the Consulate)

  2. A Serbian ID card or passport (if you have one)

  3. A passport or photo ID card from Australia/NZ

  4. Pension Fund (PIO) form (if you have one)

a) Bank transfer to the Consulate’s account

b) Cash payment at the Consulate counter

? Some banks charge additional fees for international transfers—especially from New Zealand. These costs are not included in the consular fee.

? Applications without proof of full payment will not be accepted.

Return postage

  • Enclose a self-addressed, trackable return envelope with prepaid postage

  • If you do not provide a return envelope, postage fees are:

    • AUD 15 within Australia

    • AUD 35 for international postage (e.g., New Zealand)
